Case Management

Provides problem solving interventions that assist the family to function effectively.

Along with the family, Case managers produce a positive support action plan in order to supply the services to meet specific needs and promote self sufficiency.

The case managers provide services to families in the community and the categories included but are not limited to the following:

  • Education ( ie. school issues, registration etc.)
  • Adult Education (GED/ESL)
  • Housing Referrals ( ie. rental, shelters, etc. )
  • Employment Referrals
  • Health Services
  • Emergency Food Referrals
  • Emergency Clothing Referrals
  • Furniture Assistance Referrals
  • Registration for Instructional/Recreational After School & Summer
  • Emergency service
